As a member of AJRA for the Spring 2010 season my rower and I agree to abide by it and understand that failure to live by team rules may lead to suspension or separation from the team.

6. To Be Completed by Varsity and Novice Rowers Only: My rower and I understand that consistent practice attendance is the most important factor in improving my rowing and the rowing of my team. With this fact in mind I will strive for perfect practice attendance. Because missing practice hurts not only me but also the members of the rower?s boat, we understand that a lack of attendance at practices creates a burden for the team. Therefore, we understand that if the rower?s practice attendance is lower than what is specified below, then the rower will be allowed to attend but not row at a regatta. Because every practice is important and every missed practice impacts the team, then we understand that there is no such thing as an excused absence from practice. US Rowing Waiver Form
Physician's Fitness Verification Form
This needs to be completed by a Physician, Physician's Assistant, or a Nurse Practitioner. This needs to be received at Registration or on the first day of practice. Forms not received will render the rower ineligible to practice or row at events until the form is received, with no adjustment to fees, or receipt of a refund. This form is valid for one year and is kept on file by AJRA from season to season. If the form has expired since the last season, a new form will need to be submitted.
Medical Authorization Form
This form gives basic information on the medical history of the rower and any special conditions or concerns, and allows AJRA to obtain emergency medical care for your rower at a hospital or other medical facility in the event of injury or illness at a rowing practice or event when a parent or guardian is not available. Please include both your Insurance Carrier and Policy Number, and attach a copy of the insurance card as well.
